
Permission Guidelines
As the author, it is your responsibility to obtain all necessary permissions for
copyrighted material. Permissions must be obtained from the original copyright
holder, usually the publisher, even if you are the author of the original material. Your
manuscript is not complete until all permissions are received by us. Failure to obtain
and submit all permissions will result in delay of publication of your work.

Use it to request permission or as a guideline when generating your own request
letters. Some publishers provide online permission request forms, some grant
permissions by email, others may grant written permissions only.
When you have received the permission grants, please mail or fax all the originals to:
